PSM Solutions is partnering with healthcare provider organizations who are delivering care consistent with the APhA Foundation’s tested and validated process model that includes the PSMC for Diabetes. Each of the partners below is available to contract directly with self-insured employers and other healthcare payers to deliver high quality clinical services that are tailored to meet each beneficiary’s needs. As we work to develop customized solutions for each payer, we will gain an understanding of your needs and recommend the care delivery partner who can best meet the geographic dispersion and health requirements of your population.
